A proven, CDC-recognized lifestyle change program to help prevent or delay type 2 diabetes.
Did You Know? Over 96 million American adults have prediabetes — and more than 80% don’t know it. The good news: type 2 diabetes can often be prevented with small lifestyle changes.
Diabetes on Track
Diabetes on Track is an initiative to improve diabetes care and prevention in rural Nebraska. If you have prediabetes or think you may be at risk, you are not alone—1 in 5 Nebraskans with diabetes are not aware they have it.
What is the difference between prediabetes and diabetes?
Prediabetes:
Prediabetes is a serious condition that can lead to Type 2 diabetes. It is when your blood sugar levels (or glucose) are higher than normal but not high enough to be called diabetes.
A person with prediabetes can get rid of it with lifestyle changes. Learn more about prediabetes or diabetes with our navigators, call 402-375-2200.
Diabetes:
To simplify, diabetes is when the body cannot control blood sugar levels, also known as “glucose”. Glucose is the energy that comes from the food we eat. To “active” or manage this energy, we need a hormone called insulin that travels from the pancreas.
A person with diabetes may struggle to break down the sugars in the body due to not having enough or any insulin.
