Diabetes on Track
Diabetes on Track is an initiative to improve diabetes care and prevention in rural Nebraska. If you have prediabetes or think you may be at risk, you’re not alone—1 in 5 Nebraskans with diabetes aren’t aware they have it. Most new adult diabetes cases (95%) are Type 2, which often develops from prediabetes. Since prediabetes usually has no symptoms, it's important to take a quick risk assessment to see if you’re at risk and take steps toward better health.

An A1C test is a simple blood test that helps healthcare providers:
- Detect prediabetes.
- Diagnose Type 2 diabetes.
- Monitor how well your diabetes management plan is working if you have Type 2 diabetes.
Diabetes on Track is made possible thanks to The Diabetes Care Foundation of Nebraska to the University of Nebraska Foundation and the University of Nebraska Medical Center.
